Based in Surprise Arizona, Blue Mountain is an agency committed to delivering individualized attention and tailored services to children and adults with developmental disabilities.
Our programs and highly qualified providers focus on increasing independence by addressing the physical, cognitive, emotional and social needs of our participants. We serve as a resource to families and our community.
Services
We hire and train qualified professionals to provide respite, habilitation and attendant care to children and adults with special needs. Services are delivered in the member's home and community. Goals are developed to help members reach their full potential and detaled monthly reports are mainted to document progress.
Adult Day Program - Our day treatment and training program provides participants with the skills they need as adults with special needs. Goals will focus on money management, socialization, daily living skills, recreation, leisure and community integration. Daily activities include arts and crafts, cooking, games, decision making, planning, community outings and volunteering. The program features a low ratio of approximately 1:4, transportation provided.
Program hours are 7:30 a.m. – 3:30 p.m. Monday – Friday
Our DTT and DTS programs are designed for children and young adults ages 7-21 with developmental disabilities. The program offers a variety of leisure, learning, socialization opportunities as well as community outings. Activities include games, crafts, cooking and social groups.
